Webinar series covers integrating health and social care

Bridging the Sectors: Integrating Health and Social Care, a series of webinars hosted by the American Hospital Association (AHA) Center for Health Innovation, will explore ways hospitals and health systems can work with community-based organizations to better coordinate care for patients with complex needs and drive improved outcomes. The first 90-minute webinar will explore how health care providers and community organizations are using both community- and individual-level indicators to gain a fuller understanding of patients’ needs.
