AHA podcast discusses delivering care to high-need communities
In an American Hospital Association podcast, Iris Lundy, RN, vice president of equity at Sentara Health, discusses how the Norfolk, Va.-based system is delivering accessible and high-quality health care to high-need populations. Sentara offers neighborhood clinics and mobile vans for people without transportation. It also partners with trusted community organizations and uses a geographic information system to help identify populations with the greatest needs and insufficient access to health care and community services. For example, Sentara used GIS to identify specific neighborhoods in which women who had not gotten recent mammograms. Sentara Health then provided education about breast cancer at trusted organizations and brought mammography services to the neighborhoods.
