Joint Commission issues list to reduce diagnostic errors
The Joint Commission this week released a list of 10 practices hospital leaders can pursue to reduce diagnostic errors. Published in The Joint Commission Journal on Quality and Patient Safety, the list includes organizational leadership building a “board-to-bedside” accountability framework and implementing a just culture and psychologically safe environment for diagnostic safety. “The list is thoughtful and clear, and we were particularly delighted to see a call to focus on diagnostic equity, transitions of care and the critical role of patients and families on the diagnostic team and in the diagnostic learning system,” states an accompanying editorial. “The Safer Dx Checklist provides and actionable list of priorities for hospital leaders to pursue, starting now.” (Joint Commission news release, 10/25/22)
