Podcast discusses providing age-friendly care to veterans
In an American Hospital Association podcast, Kimberly Wozneak, MS, national lead for age-friendly health systems at the Department of Veterans Affairs, discusses how the Veterans Health Administration weaves four foundational age-friendly principles into its care to support the growing number of aging veterans. Teams at the VA ask patients the 4Ms: what matters, medication, mentation, and mobility. They stress that rather than add work to clinicians’ schedules, the 4Ms streamline and improve the VA’s processes and build on evidence-based practices. To build support, the VA also communicates its successes and emphasizes how age-friendly health care improves health for veterans.
