AHA to host webinar on creating age-friendly health systems

On Aug. 10 at 1:00 p.m. ET, the American Hospital Association (AHA) will host a webinar on creating age-friendly health systems. Age-friendly health systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with AHA and the Catholic Health Association of the United States. The initiative seeks to improve patient care, safety and outcomes and reduce length of stay and readmissions by focusing on the 4Ms Framework: what matters, medications, mobility and mentation. Participants will learn about the importance and impact of the 4Ms Framework, hear bright spots of how the field is integrating value-based, age-friendly care and understand the benefits of joining the Action Community.
