Hospitals invited to attend TAKEheart webinar

TAKEheart, a free, 12-month long cardiac rehabilitation initiative, will be holding an informational webinar on Oct. 8 from 12:30 to 1:30 p.m. CT. Designed by the Agency for Healthcare Research and Quality, TAKEheart will help hospitals improve health outcomes by automating referral processes and providing care coordination for eligible patients. Hospitals that participate in the program will receive training, coaching and tools to support them in implementing the TAKEheart approach. Interested hospitals may register to attend the webinar.
