Case study: Team-based care leads to improved value
An inner-city primary care clinic serving a large refugee population is the subject of the latest case study from the American Hospital Association’s Value Initiative. The decision by CHI Health and Creighton University Medical Center in Omaha, Neb., to make the clinic a learning laboratory for interprofessional team-based care improved value for patients, many of whom endured chronic pain and mental illness. Patients who received care in the team-based model had a 16.7% reduction in emergency department visits and a 17.7% reduction in hospitalizations. Patients also had a 48.2% reduction in charges and improved patient satisfaction. Additionally, staff engagement and satisfaction soared following adoption of the team-based care model.
