Study identifies improvements to care transitions

Nurse follow-up, pre-discharge patient education and contact with local health care services improved care transitions, a study found. Published this week in the Agency for Healthcare Research and Quality Journal of Patient Safety, the systematic review of previously published studies described and evaluated the effectiveness of transition interventions to safeguard patient safety and satisfaction during patients’ transition from hospital to home health care. The authors noted interventions ensuring patient safety did not necessarily facilitate patient satisfaction. (AHRQ Journal of Patient Safety study, 3/8/23)
