Study: Medication reconciliation toolkit reduces discrepancies
A study by researchers at multiple academic health centers found implementing a medication reconciliation intervention at the system and patient levels reduced unintentional medication discrepancies in admission or discharge orders. The Multicenter Medication Reconciliation Quality Improvement Study tested the use of a toolkit in 18 hospitals over two years. During the intervention, hospitals experienced a steady decline in their medication discrepancy rate from 2.85 discrepancies per patient to 0.98 discrepancies per patient.
