RN clinical liaison ensures smoother transitions

Health care providers at Mayo Clinic Hospital in Rochester, Minn., noted a communication gap between inpatient and primary care practices when the hospital admitted primary care patients. Leaders implemented a quality improvement project resulting in the role of an RN clinical liaison (RNCL). The RNCL aimed to connect the inpatient care team with community resources and the primary care ambulatory care teams. They also observed and improved processes to help inpatient and primary care teams support best patient outcomes. Last, The RNCL facilitated communication of pertinent information across teams, supporting timely dismissals. Overall, the RNCL model enhanced services for vulnerable populations at the hospital.
