Podcast discusses how hospitals can improve care for older adults

In an American Hospital Association podcast, Rani Snyder, MPA, a vice president at The John A. Hartford Foundation in New York City, encourages health organizations to participate in an age-friendly AHA action community starting in September. The free, seven-month, virtual action community offers health systems peer-to-peer learning and coaching to integrate evidence-based principles of age-friendly health systems into their care networks. Hospitals integrating the principles often reduce lengths of stay and re-admissions, as well as improve workforce satisfaction. Age-Friendly Health Systems are an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the AHA and the Catholic Health Association of the United States.
