Nurses provide health coaching to patients post-discharge
A nurse-led initiative at Emory Saint Joseph’s Hospital in Atlanta will provide discharged patients with one-on-one lifestyle coaching to establish and maintain healthy behaviors. During calls lasting for up to 90 minutes once or twice a week for 12 weeks, nurses may help patients to reduce smoking or better maintain their blood glucose management. They also will make sure patients properly take their medication and keep timely medical appointments. The program will track reductions in readmissions and measure the benefits of achieving attainable health goals. The hospital received a two-year, $60,000 grant to start the program.
