Nurse-led model’s inclusion in NP curriculum improves outcomes
Incorporating the competencies of a nurse-led care model into a nurse practitioner curriculum has improved care transitions for older adults, according to a study. Published in the Journal of Professional Nursing, the authors explain how the transitional care model’s evidence-based interventions have helped to better prepare NPs to engage acute care providers to more effectively manage the care coordination of older adults with complex care needs. NP students learned how to make clinical decisions to support a smooth transition from the hospital to the home among multiple health care team members. They also learned to identify and address gaps in transitional care for high-risk older adults. (Newswise release, 9/25/23)
