IHI launches age-friendly health system collaborative

The Institute for Healthcare Improvement last week launched an Age-Friendly Systemwide Spread Collaborative. The collaborative engages 30 U.S. health systems to accelerate and disseminate evidence-based, high-quality care for older adults across the care continuum. During the 18-month collaborative, the health systems will test changes to ensure the 4Ms (what matters to the patient, medication, mentation and mobility) are provided reliably and equitably as a standard practice across their systems. The hospital systems will share data, learn from each other and work to advance their own improvement aims to equitably scale age-friendly care delivery systemwide. The collaborative is the latest program of the Age-Friendly Health System initiative, of which the American Hospital Association is a partner. (IHI news release, 4/16/24)
