Health system launches care coordination center
Memorial Healthcare launched a care coordination center to boost quality and patient safety. The Broward County, Fla.-based health care system says the facility will serve as a transfer center, with nurses using real-time data across the system’s six acute care hospitals to coordinate patient transfers within the system and from other facilities into the system. The center will centralize bed placement, provide virtual observers for patients at risk for falls, offer virtual nursing for admissions intake and discharge instruction and education, and manage a centralized staffing pool for nurses and other workers. The team will use real-time information on patient volume and shift staff to high-need areas. Memorial Healthcare says the center will enable it to move patients quickly and efficiently to the correct care destination. (HealthLeaders Media article, 11/11/24)
