Health system closes health equity gap in blood pressure management
The University of Chicago Medical Center eliminated a health equity gap in blood pressure management between Black and non-Black patients through remote patient monitoring and pharmacy-assisted blood pressure management. The system used data to determine the gaps between Black and non-Black patients and hypothesized the differences were caused by its Black patient population having less access to care. After implementing the programs, the University of Chicago Medical Center monitored its data to ensure the gaps were narrowing. The system also conducts care coordination assessments within the first 24 hours after admitting patients to identify social needs it can address. (HealthLeaders Media article, 10/15/24)
