App aims to improve maternal outcomes

OSF HealthCare seeks to improve prenatal and postpartum outcomes for patients with Medicaid insurance through its OSF OnCall Connect app. The Peoria, Ill.-based health system uses the app to connect patients to a nurse immediately. The app offers chronic condition monitoring, health and wellness education, and pregnancy care and postpartum support, enabling users to access specially trained nurses and advanced practice providers. Patients also can access social workers, pharmacists and dieticians, as needed. Since launching in August 2023, 970 women have signed up for the app, and 500 are active users. OSF HealthCare also has used the app to provide patients with transportation to appointments. (HealthLeaders Media article, 11/13/24)
