Focus groups identify barriers to implementing transitional care

Nurse researchers have identified 25 barriers to implementing care coordination and transition management (CCTM) in practice settings across the health care continuum. The researchers conducted a series of focus groups and used software and line-by-line reading to analyze transcripts of the meetings. Their paper discusses the barriers to CCTM and recommends strategies for overcoming them and supporting CCTM’s use in nursing practice. (Nursing Economic$ article, May/June 2019)
